Case Manager II

Position Description
Employees in this position perform a wide variety of duties related to the care, treatment, and rehabilitation of clients. These employees are aware of available community resources and function as a member of a treatment team.

Primary Duties and Responsibilities

  • Interviews and orients clients and collects information necessary to complete the intake process, not including treatment recommendations.
  • Records individual case information, updates case files, and provides case status information in order to provide a complete record of services.
  • Confers with supervisor and other team members at staff meetings, providing and/or receiving information as necessary to assure proper case management.
  • Collects and maintains data related to specific reporting requirements of grants, special client programs, and intra-agency referrals, as assigned by Clinical/Program Supervisor.
  • Provides specific community liaison services as assigned by supervisor.
  • Provides clients with Orientation to the agency.
  • Provides clients with individual and/or group counseling to facilitate case management goals/objectives.
  • Teaches interactive alcohol/drug education classes and provides other learning opportunities for assigned clients.
  • Completes urine specimen collection and documentation per agency collection procedures.
  • Performs other reasonable and related duties as assigned.

Minimum Training and Experience

  • Requires Bachelor's Degree in field related to Human Services from an accredited college or university, which include but not limited to: Social Work; Clinical Psychology; Counseling; Rehabilitative Counseling; Counseling Education; Education; Criminal Justice; or Ministry and at least two (2) years full-time experience in mental health or addictions treatment setting or certification as an Addictions Professional (CAP, CAAP, CAPP) by the Certification Board for Addiction Professionals of Florida. The alternative requirement is at least four (4) years experience in addictions and (2) years working as a Case Manager I.
  • Requires participation in a staff training program.
  • Requires knowledge of addictions treatment and/or mental health treatment programs and supportive services.
  • Requires general understanding of the causes, nature, and treatment of substance abuse and/or mental health problems.
