Case Manager II

Position Description
Employee’s primary function will be to assist in the facilitation of transitions for applicants to the program and the admitted Persons Serve between external and internal facets of the treatment team/referrals/support system. Incumbent will troubleshoot & manage the Tri-County telehealth network to facilitate user & end-user access, scheduling and utilization of this network. This employee will research available community resources and functions as a member of a treatment/support team.

Primary Duties and Responsibilities

  • Serve to work as a liaison and advocate between Tri-County Human Services’ programs and community referral sources.
  • Attend all persons served staffing.
  • Collaborates efforts to assist individuals and/or their support system through the application process to a treatment program.
  • Instruct and review with the person served the Tri-County telehealth network to assure access and effective utilization of the system between the person served and a direct service provider.
  • Effectively develops a mechanism to enable protected information to be shared with providers/referrals/involved individuals to address and meet the identified needs of the individual seeking assistance.
  • Efficiently targets treatment resources and interim or alternative interventions that will assist applicants or individual seeking services with Tri-County Human Services.
  • Engages those individual seeking services to maintain verbal and/or telehealth contact during the interim of the targeted resources. The verbal and/or telehealth contact is to occur minimally once a week. Assess if the individual seeking assistance is following through on the suggested interventions and/or resources. Incumbent’s role is to provide positive regard, guidance, and encouragement.
  • Discharge data entry into Credible computer system, as well as ongoing weekly review of status of person serve who have been deemed “High Need High Utilizer” (HNHU).
  • Organization and documentation of each individual’s case information including updating case files and providing relevant case status information in and updates.
  • Performs other reasonable and related duties as assigned.

Minimum Training and Experience

  • Bachelor’s Degree in field related to Human Services from an accredited college or university, which include but not limited to: Social Work; Clinical Psychology; Counseling; Rehabilitative Counseling; Counseling Education; Education; Criminal Justice; Ministry and at least two (2) years full-time experience in mental health or addictions treatment setting or certification as an Addictions Professional (CAP, CAAP, CAPP) by the Certification Board for Addiction Professionals of Florida preferred. The alternative requirement is at least two (2) years experience in addictions and (2) years working in a Substance Abuse / Mental Health of Dual Dx Treatment Program.
  • A strong technical background (Computer experience/knowledge) preferred.
  • Requires participation in a staff training program.
  • Requires knowledge of addictions treatment and/or mental health treatment programs and supportive services.
  • Requires general understanding of the causes, nature, and treatment of substance abuse and/or mental health problems.
